Physician Referral Form
Use this form to request an evaluation from your pediatrician. A pediatrician referral provides us with important information such as diagnostic codes that we can include on any invoices for seeking reimbursement with insurance
Some clinics have their own referral/prescription forms for speech therapy. We can accept their form, our form, or no form at all. While this form is NOT required to receive services, it may help you to successfully submit claims for insurance reimbursement.
